A UCC Financing Statement Amendment (Form UCC3) is used to change or add to information that was previously filed on the Form UCC1. This amendment form includes continuations, assignments, terminations, and amendments. Certain combinations of amendments can be made on one amendment form.
The Addendum (Form UCC3Ad) and the Additional Party forms (Form UCC3AP) should only be completed and submitted with the UCC3 if they are needed to record additional debtors and/or secured parties.
This UCC Financing Statement Amendment (Form UCC3) must be reproduced in a format prescribed by the Uniform Commercial Code, including printing on 8 1/2” x 11” white paper. Submittals that do not conform to the specifications will be rejected.
