To ensure accurate and complete indexing of debtor and secured party names associated with UCC records, the Secretary of State's Office will only index additional names appearing on an appropriate National addendum form (UC1Ad or UC3Ad) or the optional California Additional Party Addendum. The Additional Party Addendum can be used with either the National Financing Statement (UCC1) or the National Financing Statement Amendment (UCC3).
Effective October 1, 2001, names appearing on separately attached pages will not be addressed as elements of the submittal for indexing purposes.
These forms, along with all the UCC filing forms, can be obtained from the Forms and Fees webpage. The forms can be downloaded, filled out online, printed and mailed to the Secretary of State's Sacramento office at P.O. Box 942835, Sacramento, California, 94235 or delivered in person to 1500 11th Street, 2nd floor, Sacramento, California, 95814. Please read all instructions carefully when completing the forms.
